Tact Staff is seeking a travel nurse RN Med Surg / Telemetry for a travel nursing job in Antioch, California.Job Description & Requirements

    A discharge RN plays a crucial role in ensuring that patients are safely transitioned from the hospital to their homes, Skilled Nursing Facilities or other care settings. This role is essential in reducing hospital readmissions and ensuring that patients receive the support they need for a successful recovery after discharge.

    Job Responsibilities:

    1. Patient Assessment: - Conduct thorough assessments of patients' physical, emotional, and social needs and the active discharge order before discharge. - Evaluate patients' understanding of their diagnosis, treatment plan, and discharge instructions. 2. Education: - Provide comprehensive discharge education to patients and their families, including medication management, follow-up appointments, and self-care instructions. - Ensure that patients understand their discharge instructions and the importance of adhering to them. 3. Care Coordination: - Collaborate with the healthcare team (physicians, pharmacists, case manager, social workers, etc.) to facilitate a smooth discharge process. - Coordinate post-discharge care, including referrals to home health services, rehabilitation, or outpatient services as needed. 4. Discharge Planning: - Develop and implement individualized discharge plans based on patients' needs and available resources. - Ensure all necessary documentation is completed and that follow-up appointments are scheduled before discharge. 5. Medication Reconciliation: - Review and reconcile medications to ensure that patients have an accurate list of medications to take home. - Educate patients on new prescriptions, potential side effects, and the importance of adherence. 6. Patient Advocacy:

    - Advocate for patients' needs and preferences during the discharge process. - Address any barriers to discharge, such as transportation, financial issues, or home safety concerns. 7. Monitoring Outcomes: - Follow up with discharged patients to assess their recovery and identify any issues that may arise post-discharge. - Collect data on readmission rates and patient outcomes to help improve discharge processes. 8. Documentation: - Maintain accurate and complete documentation of the discharge process, including LDA removal, returned belongings/valuables , PIV removal, patient education and follow-up plans. - Ensure compliance with hospital policies and regulatory requirements. Qualifications: 1. Education: - Bachelor of Science in Nursing (BSN); Associate Degree in Nursing (ADN) accepted. - Active CA RN licensure. 2. Experience: - Previous experience in a Tele/Med-Surg unit or similar clinical setting. - Experience in discharge planning or case management is a plus. 3. Skills: - Strong clinical assessment and critical thinking skills. - Excellent communication and interpersonal skills to effectively educate and support patients and families. - Compassionate and patient-centered approach to care. 4. Knowledge: - Knowledge of medical conditions, treatments, and medications commonly encountered in a Med-Surg setting. - Familiarity with community resources and post-discharge care options.
