Overview: The Physical Therapist delivers direct rehabilitation
services to patients in skilled nursing environments, concentrating
on enhancing functional abilities and overall quality of life
through individualized therapy. This role involves working closely
with a multidisciplinary team to create and execute treatment plans
based on physician referrals, while ensuring compliance with
relevant laws and facility regulations. Candidates must possess a
valid license, experience in post-acute care, CPR certification,
and strong skills in therapy documentation and patient engagement.
Environment & Patient Demographics Our facilities function as
Skilled Nursing Facilities (SNFs) that offer: Long-term care for
residents needing continuous skilled assistance and compassionate
daily support. Short-term, sub-acute rehabilitation for individuals
recovering from surgery, illness, or injury. Collaborative care
through a team-based approach involving nursing, therapy, and
medical professionals working in unison. A varied patient
demographic that includes those with orthopedic, neurological,
cardiopulmonary, and post-acute care needs. The patient mix may
differ slightly from one facility to another. As a therapy
professional, you will play a vital role in helping patients
achieve functional improvements and enhance their quality of life
in a supportive, patient-focused setting. Position Overview The
Physical Therapist provides direct rehabilitation services in
accordance with physician referrals, adhering to all applicable
laws, regulations, and organizational standards. Education,
Experience, and Licensing Requirements Graduation from an
accredited physical therapy program, holding a BSPT, MSPT, or DPT
(those graduating in 2003 or later must possess an MSPT or DPT).
Current registration/licensure in the relevant state, with an
obligation to maintain an active license in good standing
throughout employment. Preferred one (1) year of experience in a
post-acute care or similar setting. CPR certification required upon
hire or to be obtained during orientation, with the necessity to
keep it current throughout employment. Specific Job
Responsibilities Engage in community health initiatives/projects as
appropriate. Proficient in Microsoft Word, Excel, and email
communication. Coordinate with patients, families, and support
departments to effectively address patient needs. Exhibit proper
body mechanics consistently. Make independent decisions when
necessary. Familiar with practices, procedures, and regulations
governing operations in post-acute care facilities. Implement and
interpret departmental programs, goals, objectives, policies, and
procedures. Demonstrate proficiency in all competency areas,
including patient rights, safety, and sanitation. Maintain
professional relationships with colleagues, vendors, and others.
Ensure confidentiality of all proprietary and sensitive
information. Understand and adhere to organizational policies,
including those related to harassment and compliance. Exhibit
integrity and professionalism by following the organization’s Code
of Conduct and completing required compliance training. Essential
Duties Implement physical therapy treatment plans for patients.
Set, evaluate, and adjust realistic, measurable, timely, and
functional goals. Supervise and assess care provided by PTAs, Rehab
Aides, and PT and PTA students. Document care accurately and in a
timely manner. Utilize therapy software effectively and accurately.
Provide excellent customer service and maintain a positive attitude
towards patients. Assist in the safe evacuation of patients when
necessary. Demonstrate reliable and consistent attendance. Employ
reasoning skills and sound judgment in decision-making. Collaborate
effectively within an interdisciplinary team. Engage in various
physical activities such as sitting, standing, bending, lifting,
pushing, pulling, stooping, walking, reaching, and moving
intermittently during work hours. Read, write, speak, and
