Job Description How You Will Help: Family Success Coach In this
role, you will work directly with your families to provide ongoing
support, including: Interviewing and establishing rapport with your
families, including conducting assessment to identify family needs.
Orienting and educating parents on the program and providing
ongoing training, as needed Developing appropriate family service
plans. Keeping record of all services provided including
documenting all assessments, meetings, decisions and reports. Act
as a liaison with other community agencies, making referrals to
community resources when appropriate. Assisting families in
locating funding for services if possible. Working effectively
within the agency and social work community at large. Helping be a
champion of the program, recruiting families for the program.
Success Coach --- Skills, Experience & Personality Fit Bachelor---s
degree in Social Work or Psychology, and/or Sociology. Prior
experience in a case management position or equivalent experience
Knowledge of crisis intervention and the unique needs of kinship
care and adoptive families Strong knowledge of and ability to
deliver trauma-informed practice Ability to work in MS Word,
Outlook and Excel at intermediate level Proficient in using virtual
video technologies and have stable internet service with privacy to
meet with clients virtually Must meet all required health screening
and criminal record clearances. This opportunity is a remote
assignment through September 2022 and will transition to a
tremendous opportunity for a longer-term partnership in our Antioch

intensive training and coaching to enhance and hone your skills in
the youth and family practice space. You will be working virtually
