Job Description Job Description Job Title: Social Worker (MSW) Location: Napa, California Salary Range: $85,904 – $133,348 per year Position Summary: A leading healthcare provider in Napa, California is seeking a compassionate and experienced Social Worker (MSW) to support inpatient care. Under minimal supervision and guided by the Case Management Director, the Social Worker/Patient Counselor plays a key role in meeting patients' bio-psychosocial needs and facilitating access to medical and community resources. This position involves providing crisis intervention, short-term counseling, and coordinated discharge planning to ensure smooth transitions across the continuum of care. Key Responsibilities: Conduct comprehensive psychosocial assessments for patients. Deliver short-term counseling and crisis intervention services to patients and families. Develop and implement effective discharge plans tailored to each patient's needs. Collaborate with healthcare professionals and community agencies to support patient care. Advocate for patient access to services and ensure continuity of care post-discharge. Maintain accurate and timely documentation in compliance with regulatory standards. Required Qualifications: Master’s degree in social work (MSW) or Counseling Psychology . Minimum of one (1) year of experience as a Social Worker or Counselor. Preferred Qualifications: Two (2) years of experience in a hospital or acute care setting. Familiarity with discharge planning and community-based services.
